With experience in construction, utilities, transportation and all phases of the petroleum industry, we know SIF's are not anomalies. After having spent decades driving down the total incident rates, we now have the data to prove that all incidents are not created equally. We have succeeded in reducing the number of band-aids but still need the same number of coffins!
In most companies, only 20-25% of incidents have SIF potential. What that means is that even if we eliminate the other 75%, the occurrence of serious incidents and fatalities will not go down. It is time to switch our approach.
Rather than treating all incidents the same, we can focus on the 25% of incidents that have SIF potential.
The main thing is to keep the main thing, the main thing, and you can't have 500 main things.
